Here is an example of my consent forms used earlier in the year for the interviews.

Y N Would you like me to use a pseudonym for your name?
Y N Can I use the name of your business?
Y N Can I use the name/title of your position?
Y N Will you release all details of this interview to be used in my ethnographic study for the 2011 semester, even if it is posted on a blog?

This is a very thorough consent form. There are a few things on here that I did not even think of like asking them if you could post your research with their names in it on the blog and using the title of their position.
